How to make different questions

Title

Bold text that can be used to add a title or give instructions

Free text question

  • Field name – this is the name of the field where the answer to the question will be stored. In this example it is ‘Action’. It should show what the information is, as it will be used for filtering and making dashboards.
  • Requires answer – when ticked, the report will not send until this question is answered. The reporter will see an astrix (star) by this question in the App
  • Confidential data – when ticked the answer to this question will be encrypted. This means that when someone views this report in the Web portal, they will see a row of stars as the answer to this question unless they have the permission ‘view confidential’
  • Keyboard appearance – you can select from Free text, Digits, Phone number or Email address. Each has a different keyboard with an appropriate layout to make answering the question easier.

Note: If the answer will be a number, that is added to other numbers to give a total for the week/ month, then this must use a Digit keyboard.

  • Text of question – this is what the reporter will read in the mobile App
  • Placeholder – this text can prompt the reporter about the type of answer you are looking for. This text will appear in the text box in the mobile App and the reporter will type over it.
  • Help text – This can give further guidance about how to answer the question. When you add Help Text, a blue ‘info’ text will appear by the question in the mobile app. When the reporter clicks ‘info’ he will be able to read your help text.
Free text question

Free text questions

The reporter can write anything in the answer box.

  • Field name – this is the name of the field where the answer to the question will be stored. In this example it is ‘Action’. It should show what the information is, as it will be used for filtering and making dashboards.
  • Requires answer – when ticked, the report will not send until this question is answered. The reporter will see an astrix (star) by this question in the App
  • Confidential data – when ticked the answer to this question will be encrypted. This means that when someone views this report in the Web portal, they will see a row of stars as the answer to this question unless they have the permission ‘view confidential’
  • Keyboard appearance – you can select from Free text, Digits, Phone number or Email address. Each has a different keyboard with an appropriate layout to make answering the question easier.

Free Text question

Photos

Take up to 3 photos using the camera in the mobile device. You may be able to add photos taken earlier and stored in your photo library.

  • Field name – this is the name of the field where the answer to the question will be stored. In this example it is ‘Photos’. It should show what the information is, as it will be used for filtering and making dashboards.
  • Allow photos from user library –
    • When ticked this will ask the reporter to grant access to their photo library. The Reporter will be able to take photos at any time which will be stored on their mobile device and then attach them to a report later. Note: this means that you are encouraging people to take photos using their mobile which they can look at later and may contain sensitive information.
    • If this box it not ticked, the mobile app will only allow the reporter to use the App to take pictures. The photo is not stored on the mobile device. Once the report is sent, the photo can only be viewed in the web portal by people with the View and Actions permission.

Note: Allowing photos to be attached from a photo library encourages people to take and store pictures on their mobile device which may contain sensitive information. To prevent this, and as default, this box is not ticked.

Photo question

Select one option from a range of suggestions.

  • Field name – this is the name of the field where the answer to the question will be stored. In this example it is ‘Location’. It should show what the information is, as it will be used for filtering and making dashboards.
  • Requires answer – when ticked, the report will not send until this question is answered. The reporter will see an astrix (star) by this question in the App
  • Text of question – this is what the reporter will read in the mobile App
  • Help text – This can give further guidance about how to answer the question. When you add Help Text, a blue ‘info’ text will appear by the question in the mobile app. When the reporter clicks ‘info’ he will be able to read your help text.
  • Possible values- list all the possible values. To add another box press +, to remove a box press –

Note: when you are adding forms in different languages, add the same options in the same order. This will keep the data correct when it is collected in the dashboard.

Checkbox:

Select one or more options from a range of suggestions.

Yes/no:

The default position is No. If the answer is Yes, click the switch to green (Apple) or select the tick box (Android). This question allows logic, where additional questions reveal on answering ‘Yes’.

Field name – this is the name of the field where the answer to the question will be stored. In this example it is ‘Corrected immediately’. It should show what the information is, as it will be used for filtering and making dashboards.

Hide this many question if the answer is No – In this example there is 1 hidden question, but it is possible to have any number.

  • If the answer is no, the reporter continues onto the next question
  • if the answer is yes, 1 hidden question appears for the reporter to answer

As default, the number of hidden questions is 0. In this case, the question acts as a yes/no tick box.

The hidden question will be the one which is directly after the yes/no question in the App design simulator

Signature:

The reporter can sign in a box using their finger. Make the box bigger by changing the phone to landscape (horizontal). The signature is stored as a pdf file.

Phone location data is collected, but no question appears in the mobile form.

Date and Time

click to see a clock. The current time shows by default, and you can leave this or choose a new time

Date – click to see a calendar. This shows the current date, but you can change this.
